In the event I am unable to attend the annual meeting, I (the undersigned), a member of the N.Y.S. Construction Industry – Safety Group # 469 that is underwritten by the New York State Insurance Fund hereby authorize Barry Lovell of Lovell Safety Management Co, LLC as Group Manager to hold our proxy and vote on our behalf at the scheduled meetings. The undersigned acknowledges that the Group Manager is eligible to serve as a proxy on our behalf and agrees that the Group Manager may vote on all matters presented to the general membership, including those in which the Group Manager may have an interest, including the election of Executive Committee members. I understand and acknowledge that the Executive Committee will act in accord with the powers granted to them under the group rules, including the appointment and compensation of the group manager. This voting proxy revokes all previously issued proxies that may have been authorized by the group member prior to the date of this voting proxy. It shall remain valid for this and any future meetings until withdrawn or until the member issues a superseding proxy.
